What did the system allow — and what would I change?
Pick a recent setback. Write three lines: what happened (facts only, no blame), what the system or process allowed to go wrong, and one specific thing you'd change if you ran it again.
Something went wrong and you want to extract learning rather than just moving on or assigning blame.
The wound is still raw and reflection would spiral into self-criticism rather than analysis.
Why it works
Blame kills learning. Asking "what did the system allow?" redirects attention from people to conditions, which is where repeatable fixes live.
Blame feels productive but kills learning. When you ask ‘who messed up?’, the answer is always a person, and the fix is always ‘try harder’ or ‘be more careful’ — neither of which changes anything structural. When you ask ‘what did the system allow?’, the answer reveals pressure points: unclear handoffs, missing information, time pressure, conflicting incentives. These are conditions you can actually change. The one-change rule keeps the autopsy practical — you’re not redesigning everything, just removing the single biggest contributor to this specific failure.
